Acceptance Form

New Applications November 2021 onwards:

Once you have received the offer of a place for your child you will then be able to complete your Acceptance Form and Medical Form. These will be sent out with your offer letter.

Please complete all fields as indicated on the forms and then save it as your son/daughter's name and email back to registrar@ellesmere.com

These forms should be signed by both parents and returned to the Admissions Office, making sure that you also include the following;

· A recent passport style photograph of your child

· Payment for your deposit

· A copy of your child's most recent school report

· The signed Acceptance, Medical and Music Tuition Forms
